Efficacy of ‘Trushanadi Takram’ in Udara( Ascites) with special reference to S.G.P.T, S.G.O.T and serum proteins.



Title:- Efficacy of ‘Trushanadi Takram’ in Udara( Ascites)  with special reference to S.G.P.T, S.G.O.T and serum proteins.

Dr. Bhalchandra R. Mahamuni had submitted his dissertation to Pune University for the P. G. degree M.D. (KayaChikitsa) in the year 2007. His guide was Dr. Sadanand Deshpande & Research center was Tilak Ayurved Mahavidyalaya, Pune, (M.S.) India.

          Dr. Bhalchandra R. Mahamuni in his research work tried to assess efficacy of ‘Trushanadi Takram’ as agnivardhana Chikitsa (Apetiser) in Jatodak avastha of Udara (Ascitis) with special reference to S.G.P.T, S.G.O.T & serum proteins.

          30 patients were selected for clinical study. Age group between 20 to 60 yrs was preferred.

          Drug was prepared as follows-
i)                   Sunthi ( Zingiber officinale) – root steam 1 part
ii)                Pippali ( Piper longum) – fruit 1 part
iii)              Maricha (Piper nigrum) – fruit 1 part
iv)              Yavakshara churna 1 part
v)                Saindhav churna 1 part
vi)              Takra (Buttermilk) 40 ml.

Trushanadi churna – 5 gm & Takra 80 ml. Dosage schedule was designed as 40 ml medicated buttermilk twice a day for 21 days. Route of administration was oral. Criteria for assessment were as follows-
1)              Adhmana – Adhmana was measured by measuring tape at three levels. a) Abdominal girth 8 cm above umbilicus, b) on umbilicus. C) 8cm below umbilicus.
2)              Bipadal oedema – was also measured by measuring tape at three levels a) 6 cm from the tip of the toe, b) at the level above ankle, c) 2 cm below knee joint.
3)              Abhyavarana Shakti – i) grade 0 = intake upto 1liters, ii) grade 1 = intake 1 to 2 liters, iii) grade 2 = intake upto 2 to3 liters iv) grade 3 = intake above 3 liters.
4)              Jaran shakti – liquid diet was given & digestion time required for the consumed diet at what time patient was hungry & had udara laghava was assessed i) Grade 0 = none, ii) Grade 1 = upto 12 hrs, iii) Grade 2 upto 6 hrs & iv) Grade 3 = upto 3 hrs.
5)              Samata – was observed on jivha. Photograph of jivha was taken for easy assessment & gradation was given i) Grade 0 = severely white coated tongue, ii) Grade 1 = moderately coated, iii) Grade 2 = mildly coated & iv) Grade 3 = Nirama.

Objective criteria- S.G.O.T., S.G.P.T. & Sr. Protiens levels.

Study was concluded as follows-
When 40 ml Medicated Buttermilk ,processed with Trushanadi churna was given to Patients of Ascites ,remarkable improvement was seen in various symptoms with reduction of SGOT & SGPT levels .

          Patients showed reduction in duration of jarana by three hours. Reduction in samata was observed from grade 3 to grade 2 means mildly coated. There is average weight reduction 2 kg after takrapana. In other subjective criteria like abdominal distension, bipedal oedema, udara shool, malasanga, nidravaiparyaya patient showed significant reduction. S.G.O.T., S.G.P.T. levels were reduced with increase of Sr. protein level. Trushanadi takra was more useful in kapha vataja udara. No side effects of drug were observed.
