To study of efficacy of shankha bhasma in chhardi(Vomiting)

Title:  To study of efficacy of shankha bhasma in chhardi(Vomiting)

Dr. Swati Hari Kulkarni had submitted her dissertation to Pune University in 2002 for the P.G. degree M.D. (Kaya Chikitsa). Her guide was Dr. Shrikant Wagh & Research center was Tilak Ayurved Mahavidyalaya, Pune (M.S.) India.

Dr. Swati Kulkarni in her research work tried to study the efficacy of Shankha Bhasma in vomiting (Chhardi). For this study she had enrolled 40 patients having age between 17 to 60 years, absence of severe dehydration/ circulatory shock, absence of severe abdominal pain and loose motions, duration not more than 7 days, non pregnant women and absence of high risk disease. The severity of features were graded as absent/normal = 0, mild =1, moderate =2 and severe = 3. The score of gradation of features was termed as severity index.

Study was carried out in two groups.

Group A – Patients treated with 500 mg of Shankha Bhasma.

Group B – 500 mg of rice powder (Placebo) given to the patients.

Patients were administered a tablet of identical nature in the dosage of 1 tablet 3 times a day along with non boiled tap water. In acute phase tablet was administered two hourly followed by thrice a day. The diet consisted of rice and moong dal or light diet

Severity index in cases of vomiting depicted as follows –
i)                   Frequency of vomiting – grade 0 = No vomiting, grade 1 = 1 to 3 times/day, grade 2 = 4 to 7 times/day, grade 3 = more than 8 times/day.
ii)                Consistency of vomitus – grade 0 = no vomiting, grade 1 = semisolid, grade 2 =fluid –homogenized vomitus, grade 3 = water only fluids with few particles of mucus. Severe water loss amounting to hypotension and coma.
iii)              Abdominal pain – grade 0 = absent, grade 1 = mild bearable, occasionally felt a sort of dull ache, grade 2 = moderate bearable, continuous, grade 3 = severe unbearable.
iv)              Tenderness – grade 0 = absent, grade 1 = mild, abdominal palpation bearable, grade 2= moderate, abdominal palpation unbearable as observed on facial expression such as lacrimation. Grade 3 = severe, abdominal palpation impossible, rigidity.
v)                Anorexia – grade 0 = absent, feels taste, grade 1 = mild can eat the whole food supplied does not feel taste. Grade 2 = moderate, does not eat the whole food supplied, does not eat in between meals. Grade 3 = severe features such as nausea vomiting etc, before, during or after food.
vi)              Weakness – grade 0 = absent, grade 1 = mild, can perform almost all activities with minimal difficulty, grade 2 = moderate, feels exhausted after routine activities, grade 3 = severe can not perform bodily activities confined to bed, hypotension, coma.
vii)           Nausea – grade 0 = no nausea, grade 1 = nausea after food, grade2 = nausea during food, does not eat the whole food supplied, grade 3 = vomiting after nausea.
        Total score – severity indices were as follows 1 to 7 = mild, 8 to 14 = moderate, 15t0 21 severe.
          Study was concluded as follows

Tab Shankha Bhasma 500 mg TDS gives good results in vidagdhavastha ( Vomiting due to Indigestion ) .

The relief in anorexia was found due to deepana & pachana properties of shankha bhasma. In acute condition shankha bhasma was administered two hourly, which reduced the frequency of vomiting.
